What insurance do you have? The first question I ask patients when they come in the door at Oxygen Plus Medical Equipment is what insurance do you have? Often times they hesitate or question what they have. I always ask to see the insurance card. Looking at the card is always the best to insure correct information. It amazes me that most often they have no idea what coverage they have. The biggest problem is that they don't not only know what coverage they have but where they can get supplies or treatment. Sometimes, people are enrolled in an HMO that dictates where they can go and receive services and treatment. It is also common that the person has no idea what their copay is or deductible is. Not only is it imortant to know your benefits prior to receiving treament or services but it's essential to know where you can go. Insurance doesn't always cover all services or treatment. Most people over 65 years old have Medicare or a Medicare Advantage plan.&lt;br /&gt;However, Medicare only covers 80% of services under Part B benefits. So additional secondary insurance is necessary to cover the remaining balance incurred. I have shopped for family, shopped for charity, read Christmas cards and thought about sending Christmas cards. I often wonder why do we do all of this. Today, I was sitting in my store and a customer came in and gave Ronda a small box with my name on it. I said to Ronda my respiratory technician whats that? Ronda said, "remember Mary( changed the name for privacy)?" I said of course she went on to tell me that Mary had dropped off those gifts for us. Because we had been so helpful to her and she wanted to say thank you! At that moment I realized that small act of kindness and appreciation is what encouraged me to enter the health care profession and keeps me in it today. That was more than an act of kindness. It was thoughtful, genuinely nice and so sincere. It had nothing to do with the actual gift it was the thought and effort behind it. I guess that is why we shop often too much at Christmas, send cards, etc... We want to make others feel that small amount of kindness. So I guess when I'm feeling unappreciated I need to remember Mary! Consider" paying it forward" and brighten someones day. It really makes a difference. If every person took a few minutes to "pay it forward" can you image what the world would be like? This person had "ordered" a power chair from a company an hour away. It had been 5 weeks and they still didn't have the chair. I usually can get a chair within 7 days to a patient. The hold up is normally the physician. If it takes longer&amp;nbsp;I am usually waiting on the physician to prepare necessary documents. I explained the procedure to the prospect.&lt;br /&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; &amp;nbsp;I also told him that&amp;nbsp;I do not do short term rentals except for existing patients. Also, if he wasn't satisfied with the current arrangement I would be more than happy to help him. But, I wasn't trying to force him into anything because after all it was his choice where to receive equipment. However, he stated his doctor referred him to this medical equipment company. &lt;br /&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; There is several concerns with this situation. First of all, the patients has the right to choose where to receive care and equipment. Medicare is very clear on this policy. This is not tolerated or endorsed. There is nothing wrong with giving a &lt;strong&gt;"list"&lt;/strong&gt; of references or referral sources but it should be made clear that the patient has the right to choose. I can understand with smaller pieces of equipment the desire to buy long distance however, for longer items such as a scooter or power chair it is a good idea to look local first. After all, equipment can break and need minor repairs. &lt;br /&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; So, I would ask myself is that company that is far away going to respond fast and efficiently? I often receive long distance opportunities if I know servicing is going to be difficult I often refer them to a local company. After all, the&lt;em&gt;&lt;strong&gt; PATIENT&lt;/strong&gt;&lt;/em&gt; is first. At Relay, teams of people camp out at a local high school or park. Each team is asked to have someone on the track all times during the event. Because cancer never sleeps, Relays are overnight. &lt;/div&gt;&lt;br /&gt;&lt;br /&gt;&lt;div&gt;The next question is always what happens at &lt;span style="color:#330099;"&gt;Relay For Life&lt;/span&gt;? There is an opening ceremony, luminaria ceremony, fight back ceremony and closing ceremony. There is entertainment during the day, kids activities and fundraising opportunities throughout the event. Are you at risk? If you say yes to any of these following questions you are at risk....Are you 45 years of age or older? Do you have a parent,brother or sister with diabetes?&lt;br /&gt;Are you African-American,Hispanic,or Native American? Are you overweight? Do you get too little physical activity? Do you have high cholesterol?&lt;br /&gt;&lt;br /&gt;So if you ansered yes to any of the above questions you need to take action NOW.&lt;br /&gt;Here are a few things to help. The first thing is increase your daily physical activity. You want to start slow and increase activity daily. The normal amount of activity is usually 20 minutes. This can be walking, running or even simple cleaning around the house or yard. The next thing is eat healthy. Try eating a well-rounded breakfast it's the most important meal of the day. Continue eating every 4-5 hours and eat 5 or more servings of fruits and vegetables daily. Eat whole fruits instead of fruit juice. Another easy change is switching to lowfat dairy products. If you have high blood pressure you want to talk to your doctor immediately. Limit salt intake and exercise will help along with any medication that is prescribed. Along with treating high blood pressure you want to consider your cholesterol also. Decreasing intake is important in managing cholesterol. The last concern is stress. Managing stress is vital to minimizing health problems. Finding ways to relax is key. Often exercise can help as well.&lt;br /&gt;&lt;br /&gt;Many of us are at risk for Diabetes 2,but by taking action, we can prevent the disease. Most people at some point in their life can relate to neck or back pain. As people age it's even more important for older adults to stay mobile. By keeping flexible and mobile it can help prevent injuries. Often neck pain can be caused by another problem and also lead to other more serious problems. Neck and back pain can lead to long-term ailments and be debilitating. &lt;/div&gt;&lt;br /&gt;&lt;div&gt;     Studies show 2 out of 3 Americans will suffer from neck or back pain. Often the two problems are related. Preventative care can be crucial. There are several factors that lead to neck and back pain. For starters poor sleep, incorrect posture and performing tedious functions can aid to pain and other injuries.&lt;/div&gt;&lt;br /&gt;&lt;div&gt;     It's often that medication is given to relieve pain. Anti-inflammatories, pain relievers and muscle relaxers are among the most popular to relieve pain. For 2011 medicare recipents have a $167.00 deductible. What does that mean? Starting January 1, 2011 the recipent or patient will have to pay out of pocket $167.00 before any office visit or medical expense is covered. Also, medicare only pays for 80% of the charges. If there is no secondary insurance than the patient would be responsible for the 20% remaining balance. If you have private insurance than other deductibles and co-pays may apply. Most of the ethical agencies would like to see regulation to protect our elderly.&lt;br /&gt;There are currently bills in Lansing to enable our elderly to be left unprotected by allowing "caregivers" to list themselves as if they're credible but with no protections in place.&lt;br /&gt;Not all private caregivers are good, or bad, but there are many people that are advertising as caregivers who are ex-cons, have been terminated from agencies, or can't get hired by agencies because of their backgrounds. Some are just preying on the elderly.&lt;br /&gt;Just recently a woman who hired a "caregiver" from Craigslist, was murdered by that so called caregiver, who turns out to have had a long history of psychiatric problems and criminal charges.&lt;br /&gt;WE MUST speak up to Lansing to protect some of our most vulnerable citizens!  We just can't allow a bill that would be like a virtual State of Michigan's craiglist where no standards or safety measures are put in place! SEIU has hired  two large PR firms  to convince senators and their constituents that this is a good bill.The bill would create the Michigan Quality Community Care Council (MQCC)Act allowing,  by law, MQCC to maintain a registry of caregivers for the Home Help program.  The caregivers on this registry will become SEIU members.   It is unclear who will be their employer and who, if anyone will pay the employment related taxes and insurance for these caregivers   HB 6195 doesn't ensure quality care,  doesn't provide adequate protections, and it does not create a level playing field!The National Private Duty Association (NPDA) has worked hard for many years to bring high quality standards to the home care industry. HB 6195 doesn't ensure quality of care of Michigan's most fragile seniors and disabled citizens.  The following are minimum standards for NPDA members that are not required by HB 6195.  A bureaucrat testified at committee that they do some of these things, whether that's true or not, all of the following should be required in the bill. In-home assessments.  NPDA industry standards require that an in-home assessment be done before a caregiver is referred to determine the care needs and ensure that the caregiver's training and abilities match the needs of the client. In addition, regular in-home follow-up should be done, to ensure the quality of care and make any necessary changes in the care plan. HB 6195 should require in-home visits.Criminal background checks.  NPDA requires that all individuals hired as caregivers pass a comprehensive criminal background check.  HB 6195 should require background checks.Training requirements. NPDA requires that all caregivers receive a minimum of 16 hours of training before they are allowed to provide care. Topics of mandatory training should include areas like: Red Cross first aid and CPR, Basic Care Giving, Senior Safety, Senior Activity, Alzheimer's Training, Personal Care Training, and Transferring including Hoyer lifts. HB 6195 should require minimum training standards for caregivers.Payment of all taxes and insurance.  Many seniors are shocked to learn that their caregiver is considered an employee by the IRS and other governmental agencies and as such are responsible for employer share of taxes.  NPDA standards require that all employer related taxes, including FICA and Unemployment Insurance, as well as workers compensation are paid in full. However, it still does not go far enough to meet the above suggested standards and there is no guarantee that it will not require the unionization of workers.We are urging everyone to please contact your Senators and tell them  you do not support SB 731 or HB 6195.  Urge them not to support these bill either.  These bills  don't ensure quality care and don't provide adequate protections!  They are  not good for our seniors and puts them at risk. The statements were all about the need for home health care and the financial advantages to the taxpayer of being taken care of at home over institutions such as nursing homes. Without the so-called "forced unionization" issue, the legislation is about making sure those who work in Michigan's home healthcare industry are qualified and undergo background checks. A news release circulated at the press conference sought to "fact check" what it referred to as factual errors issued by a group called National Private Duty Association. "Representative John Walsh, who is a lawyer, has looked the bill over up and down and says there's nothing that indicates any kind of forced unionization," Johnson responded. MIRS asked if attempts to amend the bill with wording to prevent forced unionization would be adopted or quick gaveled down in the House. "That's a leadership question," Johnson said. "Attempts to amend bills are always in play on the floor." MIRS asked if the timing of the legislation's movementat this timewas connected with efforts to get endorsements of the SEIU. "I'm glad you asked that," Johnson said. "This isn't just an SEIU question. This isn't a union issue." Johnson went on to say that the bill was supported by AARP and several other groups that aren't unions. He said that, in an election year there are a lot people politicizing almost everything. Supporters also argue claim that using the rhetoric "forced unionization" is unfair because it insinuates the SEIU election broke state law, which by all appearances, it did not. While it could be argued that the Granholm administration back-doored a bunch of employees into a union, it's never been argued the process was illegal. Also, it's pointed out that any person within the MQC3 can opt out of the union at any time, but they still must pay and administrative fee. A Walsh amendment was adopted on the House floor. It inserted language that stated the "information in the registry is confidential, is not subject to disclosure under the freedom of information act and shall be disclosed only to providers and consumers for purposes of this act." Two amendments offered by Rep. The physician cannot mandate where they receive service.&lt;br /&gt;&lt;br /&gt;In this situation the patient should research on their own where to get the equipment or care. Normally, I recommend getting several references from family,friends and other health care professionals. Another key is to call your insurance company often they have a list of accepted providers or caregivers. Using your local company is always an option also.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;If you have Medicare for primary insurance it is clearly stated by Medicare that the patient has the right to choose wherever they receive care and in no way can the provider or supplier entice the patient in any manner. The same holds true for the physician. It is NOT acceptable for the physician or caregiver to insist the patient receive equipment,care or service from one particular company. First of all, the process is usually initiated by a licensed professional.  Normally, a MD, PT or OT will recommend the need for a power operated vehicle. For a power wheelchair or electric wheelchair an evaluation needs to be performed and that licensed professional will usually recommend a specific piece of equipment. Then the physician or person who evaluated the patient will sometimes refer or tell the patient of a place they can get it. At times the patient will know of a medical equipment store themselves. Medicare and many other private insurance companies reimburse for this piece of equipment as long as the appropriate paperwork is in place and the criteria is met. Depending on the insurance criteria for reimbursement is different. Obviously, reimbursement is for the least costly alternative based on the need shown. It is important to know that not all power wheelchairs need an evaluation from a PT or OT. It depends on the model and the billing code.&lt;br /&gt;&lt;br /&gt;I always tell my patients if they feel they need a power operated vehicle of some kind they probably do. The first step is to talk to your primary physician or nurse practioner.  A power wheelchair is not always what is needed. At times, a scooter will best fit the needs of the patient. The physician can help determine that. A cluster headache is defined as a recurrent headache that occurs in"clusters". They are often the most painful and are characterized by a stabbing feeling around or near one of the eyes. Cluster headaches can last for hours or days. Usually, the pain is so intense that one can not work, stand or function at all. The current treatment for an acute attack is an injection of sumatriptan. However, frequent dosing is not recommended because of the side effects.  Cluster headaches are more common in men than women  and the age group is usually 28-30 years of age. It is becoming more common to use a high-dose , and a high flow oxygen.  A prescription is required for the oxygen.  There is no side effect or danger in using oxygen for a cluster headache. Normally, a home medical equipment company can supply the oxygen. The treatment is usually at least 12 liters per minute and the oxygen has to be delivered via face mask. For the most part insurance does not cover this benefit. Usually, the oxygen has to be used at onset of headache and if treatment is going to work the headache usually eases up within 15 minutes of using the oxygen. There are several complications to consider. They include heart disease, blindness, glaucoma, nerve damage, and kidney damage. This is often done in conjuntion with home care. Things to look at include the following.&lt;br /&gt; 1.In living area remove rugs,make larger clear paths, remove low coffee tables and remove soft furniture. Also, consider a lift device if person is unable to get up with ease. call 734-429-7444 for more info.&lt;br /&gt;2. If there are steps in the house use railings.&lt;br /&gt;3. In the bathroom use grab bars,bath seat raised toilet seat or handles and a non skid mat inside of the tub.&lt;br /&gt;4. Kitchen safety is often forgotten. Some quick ideas include using an old fashion timer that can be kept in a pocket, wear long sleeves while cooking and consider having meals delivered. Meals on wheels is an excellent alternative to cooking.  The next important point is that if you have any physical,hearing or vision issues always tell your doctor. They may have suggestions or alternatives. The last point to fall prevention is medication analysis. Falls are often related to medication. Always tell each doctor you visit what medications your on espically if you see multiple doctors. It's a respiratory disease originating first in pigs. This particular influenza virus has mutated so it is infectious now to humans. Because it originated in pigs humans do not have immunity. There has been conversation about it being pandemic. Pandemic mean"world-wide". United States is not yet involved in the pandemic. You can get the flu if someone coughs or sneezes in close proximity to you or if you touch a contaminated surface. The flu can be passed up to seven days after becoming infected of which the first two days may be without symptoms. So many may not even know they are infectious until it is to late. &lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family:courier new;font-size:85%;"&gt;Signs and symptoms are fever, cough,sore throat,body aches, chills and a temperature. The big warning is if a fever returns within 24-48 hours. The best prevention of spreading a virus is to stay home and away from other people when feeling any signs or symptoms. Other things to consider is to get plenty of rest, exercise often, manage stress, eat healthy and drink plenty of fluids.The best prevention from getting the swine flu or any other flu is to handwash as often as possible. The vaccine is developed much like the original flu vaccine. It will be available mid October. Pregnant women and children will be the first to receive it. Also available will be the nose mist. If your child is 10 years old or under they will require two shots. The first thing individuals should do is review their policy if they do not understand the coverage call the 800 number on the back of the insurance card. Never automatically assume that you have medicare just because you have a medicare card ! It is common to have medicare and then a medicare advantage policy for insurance coverage.&lt;/span&gt;&lt;/strong&gt;&lt;/div&gt;&lt;div align="left"&gt;&lt;strong&gt;&lt;span style="font-size:85%;"&gt;     Medicare advantage plans are extremly popular these days. Most of the older population have medicare or medicare advantage. These two names are NOT the same. For example, most private insurance companies have their own medicare advantage plan. What does that mean? Medicare (the government) has said that they will allow for private insurance companies to offer coverage to seniors who would normally have traditional medicare.  This allows for private insurance companies to bill &lt;/span&gt;&lt;/strong&gt;&lt;strong&gt;&lt;span style="font-size:85%;"&gt;premiums,collect copays or deductibles and then they pay the claims submitted. Obviously, this benefits everyone. People are given other choices than Medicare and it allows private insurance to "compete" in this market. &lt;/span&gt;&lt;/strong&gt;&lt;/div&gt;&lt;div align="left"&gt;&lt;strong&gt;&lt;span style="font-size:85%;"&gt;     If someone has a medicare advantage plan that means the insurance company that provides that policy is who collects the premium and pays the claims. Lets use BCBSM as an example. If someone has Medicare plus blue the person pays BCBSM for the policy and they in return pay the claim. Now to confuse things more most of these claims are billed to a contracted area of BCBSM not to BCBSM directly. The provider that is supplying the service or product  should call the provider number on the back of the card to find out where the claim should be sent. In the case of BCBSM most of the medicare advantage claims go to Dmension Benefit. In addition, these claims are paper submitted unlike Medicare claims which are all sent electronic.&lt;/span&gt;&lt;/strong&gt;&lt;/div&gt;&lt;div align="left"&gt;&lt;strong&gt;&lt;span style="font-size:85%;"&gt;      Medicare and medicare advantage plans cover similar services but not always the exact services. The policies do vary in coverage and premium price. Medicare only covers 80% of services. She wanted to talk to me about getting oxygen therapy. She stated that she had noticed a change in her energy,thoughts and was very tired all the time. She had other minor symptoms as well. She started talking to her friends and reading information and thinks that her change in status is because of the lack of oxygen. Mary called her doctor and they attributed it to something else. So she asked what do I do know?&lt;/span&gt;&lt;br /&gt;&lt;span style="font-size:85%;"&gt;     As a equipment company I suggested she talk to her doctor again. Explain the situation and her feelings. Secondly, I explained that there is a simple test that the doctor or a qualified home medical equipment company can perform. It is a 24 hour overnight pulse oximetry test. It is a device that goes on the finger and the patient wears it overnight. It measures the oxygen saturation. The HME company picks up the device  and downloads the info with special software. The test is then given to the physician and they interpret it. If oxygen is necessary the physician then has the appropriate test on hand. Gives the HME a valid prescription and the HME company can then deliver the oxygen. This can all happen within 48 hours or sooner.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-size:85%;"&gt;     Like I told Mary this test is very simple it doesn't hurt and the results are quick. Remember, if you feel something different or have an abrupt change in signs or symptoms call your physician immediately. Clear communication and persistent is the best.  Oxygen is considered a drug and a prescription is absolutely necessary. The best lesson I've learned is that we as individuals know our bodies best.
